Every Veteran deserves a safe, stable home and a chance to thrive. Our program provides personalized support to help Veterans make the transition from homelessness to housing a successful and permanent one.


SIX MONTHS OF CASE MANAGEMENT

Each participant is matched with a case manager who offers:
• Personalized support plans
• Connection to local resources
• Mental health and medical referrals
• Employment and benefit assistance
• Collaborative goal setting and problem-solving
• Minimal financial resource to secure housing essentials
• Ongoing check-ins to monitor progress


HOUSING STABILITY FOCUS

We work with Veterans who have recently secured housing and those who are already housed but at risk of losing it, to support a lasting path to stability.


WHO WE SERVE

• Veterans eligible for VA health care and have a qualifying discharge
• Veterans who have recently exited homelessness and currently residing in Pierce and Kitsap counties
• Veterans who are housed but are facing challenges that put their housing at risk
• Individuals ready to engage in support services and take steps toward lasting stability
• Veterans who are not actively receiving case management services from SSVF or VASH
